What this website collects
The appointment request form asks for the parent or guardian’s name, phone number, and email, your child’s age, a reason for the visit, a desired date and preferred time, and an optional note. It is for scheduling only.
Please do not put detailed health information in the form. Call the office instead. The form is not monitored outside office hours and is not for emergencies. For anything urgent, call 248-360-2555 during office hours or the emergency line at 248-345-6775 after hours.
When you send the form, your message reaches our team by email and a copy is kept in the website so it cannot be lost. The web server also keeps standard logs: IP address, browser, pages viewed, and the time.
How we use it, and what we never do
We use what you send to reply about the appointment, to confirm your coverage before the first visit, and to keep the records a dental office must keep. Server logs are used only to keep the site running and secure.
We do not sell your information. We do not share it with anyone for marketing. We do not use it for advertising. A member of our team reads your request, and it goes to no third party except as described under cookies and third-party services below.
Form messages and server logs are kept only as long as they are needed for scheduling, for security, and for the records we are required to keep.
Cookies
A cookie is a small file a website stores in your browser. This website sets only the cookies it needs to work: a cookie WordPress uses to check that your browser accepts cookies, and a security cookie set by our hosting provider, WP Engine, to protect the site from automated abuse. Neither one tracks you across other websites or identifies you to us.
We do not use analytics cookies, advertising cookies, or social media trackers. Nothing on this site follows you after you leave it, and we do not build a profile of visitors.
The maps on our contact and service area pages are embedded from Google Maps, and our fonts are served by Google Fonts. When those load, Google receives your IP address and may set its own cookies under the Google privacy policy. You can block third-party cookies in your browser settings, and the site will still work; only the map will be affected.
Analytics and third-party services
This website does not run analytics or advertising trackers. If that changes, this page will say which tool is used, that it records anonymous usage data, and how to opt out.
New patient forms are completed on OperaDDS, a secure service for dental offices, not on this website and not as PDFs. What you enter there reaches the office under OperaDDS’s own privacy policy, which it publishes on its site. The link to the forms is on our first visit page.
Links to Google Maps, to Google reviews, and to the AI assistants in our footer open other companies’ websites. Once you follow any link off this site, that site’s privacy policy applies.
Children, parents, and guardians
Our patients are children. A parent or legal guardian gives us the information we hold and exercises the child’s privacy rights on the child’s behalf, including every right listed below. We do not knowingly collect information through this website from a child directly.
Either parent or legal guardian may act for the child unless a court order on file with the office says otherwise. If you have a custody or guardianship order that affects who may see your child’s records, please bring a copy to the office so we can follow it.
When a patient turns 18, the rights in this notice pass to them, and the office will need their permission before sharing records with a parent.
Notice of Privacy Practices
Health information about your child may be used and shared, without a separate permission, for treatment, for payment such as billing an insurance plan, for running the office, for appointment reminders, and when the law requires it, such as reporting to public health authorities.
Anything else needs your written permission first, and you can withdraw that permission in writing at any time. That includes using health information for marketing, selling it, and most sharing of psychotherapy notes. Nothing this notice does not list happens without your say.
Our duties: keep your child’s health information private, give you this notice, follow its terms, and tell you if a breach affects your child’s information.
Your rights, questions, and changes
You may inspect and get a copy of your child’s record, ask us to correct it, get a list of the times we shared it outside treatment, payment, and operations, ask for limits on how we use or share it, ask to be contacted in a certain way or place, and get a paper copy of this notice.
To use these rights or ask a question, call 248-360-2555 or write to Dr. Naila Farooq at the office address above. You may also complain to the U.S. Department of Health and Human Services, Office for Civil Rights, at www.hhs.gov/ocr. A complaint never affects your child’s care.
We may update this policy. The effective date at the top shows the current version, and the version posted on this page is always the one in force.
